SurveyMonkey Guidelines


Getting Started

If you are IU Libraries staff and are interested in initiating an online survey using SurveyMonkey, please contact Michelle Dalmau, mdalmau@indiana.edu or (812) 855-1261.

Please consult the following for help with survey setup:


1. Login to SurveyMonkey to set up a new survey. Reference your email for the username and password.

2. Create a folder for your survey.

  • Go to "My Surveys"
  • Click on the "Manage Folders" button
  • Click on the "Add a New Folder" button
  • Gave the folder a name. If it is a departmental survey, please name it accordingly (e.g., Public Services, References, etc.) if it is a professional development survey, the folder name should be your first and last name. If you aren't sure what to name the folder, please contact mdalmau@indiana.edu for guidance.

3. Create your survey. Please reference the SurveyMonkey help.

4. Move survey into newly created folder.

  • Go to "My Surveys"
  • Click on the "Manage Folders" button
  • Select current folder as "View All Surveys"
  • Click on "Manage Folders" button
  • Find your survey and assign it the proper folder

5. Disseminate your survey via email, listservs or web site links.


Keep in Mind
  • Surveys, questionnaires, etc. are covered by federal regulations for the protection of Human Subjects. Please review the guidelines associated with the Human Subjects approval process before disseminating your survey. If you do not intend to publish the survey findings, then you are more than likely exempt from Human Subjects review. If the nature of your survey is in support of "quality improvement and quality assurance" and the findings will be shared internally, then you are more than likely exempt from Human Subjects review. If you have any questions whatsoever, please contact Michelle Dalmau or email Human Subjects directly.
  • Many of your colleagues will have active surveys. Please be mindful of this. Be careful to not accidentally delete/alter existing surveys or delete/alter folders. If a survey is accidentally deleted, please contact Michelle Dalmau immediately.
  • Do not share the username and password with others. All new surveys, including any additional surveys you may initiate after this one, needs to be documented by the UWG. Every few months, passwords will be reset. Contact Michelle Dalmau every time you need to setup a new survey.
  • With the login, you also have access to account settings and other administrative features. Please do not alter these.
  • Do not use the address book feature since multiple users will be creating surveys and disseminating the survey across a wide audience. Surveys should be disseminating outside of SurveyMonkey via email, listservs, web site links, etc.
  • Keep a text version of your survey questions as a back-up.
  • Inactive surveys will be deleted after six months to facilitate shared account management. Please archive your completed survey by exporting the data. There are many options available for export including export to database, spreadsheet and XML. Consult the help for more information. If you need additional time for processing the results, please let Michelle Dalmau know.
